How to Sign Up for Daycare

Steps:

Schedule Your Temperament Assessment: All dogs must complete a temperament assessment prior to being accepted into daycare. Contact us below to indicate your interest in daycare!
Submit your Vaccination Documents: Once your pup passes the temperament assessment, all proof of vaccinations for rabies, distemper, and bordetella must be submitted. Submit your vaccine documents below.
Submit Your Liability Waiver: We require a Liability Waiver to be completed prior to attendance. See below.

Important PlaynPup Policies:

Reservation/Cancellation Policy: We require 24 hours notice for both reservations and cancellations

Spay/Neuter: Dogs must be at least 6 months of age and be spayed or neutered to be accepted into daycare.

Flea/Tick Prevention: We request that you use flea and tick prevention for the health of all our pups.

Illness: If your dog becomes ill with a contagious viral or bacterial infection, we require a vet's letter of good health to return to daycare.

Temperament Assessment

Every dog accepted into our daycare must undergo a temperament evaluation to make sure they are friendly and a good fit with the other dogs in our playcare.
Contact us below to express your interest and we will work with you to find a mutually convenient time to meet.
Assessments generally take 20 to 30 minutes. Please allow enough time. We want to get a good feel for your dog and give them enough time to relax and get used to our space and slowly introduce them to other dogs.
If your dog's behavior is acceptable during the initial assessment, you will be approved.

I hereby acknowledge this release from liability for accidental injury or illness which my pup may incur as a result of participating in any physical activity at PlaynPup. I hereby assume all risks connected therewith and consent to participate in this daycare. I agree to disclose any physical limitations, disabilities, ailments, behavioral issues or impairments which may affect my pup's ability to participate in this program.

I authorize emergency medical care to be provided by the above named veterinarian/clinic or an appropriate altenative to be determined by PlaynPup LLC in the event that the client's named veterinarian is not available. I agree to reimurse PlaynPup LLC for any charges related to emergency care.

I agree to be responsible for all fees for services rendered, cancellation fees and other fees as agreed upon.
